What Is the I-693 Immigration Medical Exam?


The I-693 exam is required by USCIS to confirm that an applicant does not have certain communicable diseases and has received required vaccinations.


The physical includes:


  • A review of your medical history 

  • A physical exam by a civil surgeon approved by USCIS 

  • A review of your vaccination record 

  • Blood and urine tests for specific communicable diseases 

  • Completion and sealing of Form I-693 for USCIS

Civil Surgeon Exam


Our certified civil surgeon will conduct a comprehensive physical examination and review your full medical history, including past illnesses, current medications, and chronic conditions.


Required Laboratory Tests


USCIS mandates certain blood and urine tests for immigration purposes:


  • Tuberculosis screening (IGRA blood test)

    • Note: A chest X-ray may be required if the TB test is positive. 

  • Syphilis testing (RPR blood test) for ages 18–44) 

  • Gonorrhea screening (urine test) for ages 18–24) 


All required lab tests are performed on-site or by a trusted partner lab for speed and accuracy.


Vaccination Review and Catch-Up Immunizations


We will review your immunization records to verify whether you have received all vaccines required by USCIS. If you are missing any, we can administer them during your appointment.

Common vaccinations required include:


  • MMR (Measles, Mumps, Rubella) 

  • Varicella (Chickenpox) 

  • Tetanus, Diphtheria, Pertussis (Tdap) 

  • Influenza (if applying during flu season) 

  • Hepatitis B 

  • COVID-19 


Some vaccinations require multiple doses spaced out over time. If this applies to you, we’ll provide a clear schedule and follow-up appointments as needed.


Sealed I-693 Form


Once all medical and lab requirements are complete, we’ll prepare and seal your official I-693 packet for submission to USCIS. You’ll also receive an unsealed copy for your personal files.

 

What to Bring to Your Appointment


To ensure your visit goes smoothly, please bring the following:


  • A valid government-issued photo ID (passport, visa, green card, or driver’s license) 

  • Any previous vaccination records, translated into English if necessary 

  • Records of any prior TB treatment or abnormal chest X-rays (if applicable) 

  • A list of your current medications and medical conditions 

  • Your A-number, if available 

  • Any USCIS-related instructions from your attorney or case officer 

 

When to Schedule


The I-693 form must be signed by the civil surgeon within 60 days of submission to USCIS. We recommend scheduling your exam once your adjustment of status application is nearly ready to be submitted or if USCIS has requested a medical.

Frequently Asked Questions


Can I use my own vaccination records?

Yes, as long as they are official, translated into English, and include the date of each vaccine and provider information. We will review them for accuracy and completeness.

What if I don’t have vaccine records?

We can perform titers (blood tests) to check for immunity or administer catch-up vaccinations during your visit. We’ll explain your options based on USCIS requirements.

How long will the process take?

The initial appointment usually takes 30–45 minutes, depending on your medical history and lab needs. Your sealed I-693 form is typically available within 3–5 business days after test results are finalized.

Will USCIS accept my sealed form from MRHS?

Yes. All forms are completed and signed by a USCIS-designated civil surgeon, and sealed in accordance with immigration requirements.

What happens if my test results require treatment?

If we detect a treatable condition (such as latent TB or syphilis), we will explain your options and help you begin treatment. In many cases, we can still complete your I-693 once treatment has begun.

Do I need to be vaccinated for COVID-19 to pass the I-693 exam?

Yes, full COVID-19 vaccination is required under current USCIS policy, unless you qualify for a medical or religious exemption. We offer the vaccine if needed.
